Save the date! The Spina Bifida Association of NYS is proud to announce that we are hosting a Family Retreat!

This family retreat is designed for any child living with Spina Bifida aged 18 years and younger, along with their immediate family. Sign up is on a first come, first serve basis! You can reserve your spot for the weekend by paying a refundable $50, which you will receive upon arrival to the family retreat. Please note that this is a completely free weekend where we want our young families to relax and enjoy time with one another.

Waiver Agreement

In consideration of me and/or my minor child being permitted to participate in the Spina Bifida Association of New York State's Frost Valley Family Retreat, I hereby for myself, my heirs, and personal representatives assume any and all risks that might be associated with the event. I further waive, release, discharge and covenant not to sue Spina Bifida Association of New York State, its programs, officers, employees, sponsors, organizers, volunteers, or other representatives or their successors and assigns, for any and all injuries or damages of any kind whatsoever suffered by myself and/or my minor child as a result of taking part in the event and any related activities.

I have read and understand the waiver and release of liability and agree to its terms.

Adult Supervision Agreement

Frost Valley requires parent/guardian supervision of children under the age of 12 at all times.

I acknowledge and agree to comply with this requirement and accept responsibility for the supervision and safety of my child(ren) at all times during the retreat.

Photo Release Agreement

I do hereby grant full permission to the Spina Bifida Association of New York State, its agents, and others working under its authority, full and free use of video/photographs containing my or my child(ren)’s image/likeness. I understand these images may be used for promotional, news, research and/or educational purposes and may depict and/or identify him/her as a child with Spina Bifida.

I grant permission for the use of photographs and/or video of myself and/or my child(ren) as described above.
